- Proactive refill reminders: Using digital communication programs, our pharmacists can reach out to patients prior to refills through our patient portal, in addition to telephone and SMS messages.
- Refill reassessments: Patients are screened by patient care coordinators at each refill to determine if their condition has gotten worse or if any medical changes need to be addressed by the pharmacist.
- Holistic patient care: We provide comprehensive solutions for patients who need additional touch points including adherence tracking, patient follow-up, patient education programs, and medication assistance programs.

- Building customized data sets relating to patient journeys. This includes status of prior authorizations and roadblocks, dispensing information, and more. We collaborate with manufacturers so they can understand exactly what is happening with patients. Our clinician calls with patients help improve the onboarding experience, therapy, and the overall patient journey. We have dedicated resources, contacts, and subject matter experts that manufacturers can reach out to so their questions can be answered quickly, and we can step in to fill any gaps. We are nimble and adaptable and able to roll out changes quickly without long change control processes.
- Increasing speed to therapy by performing an in-depth analysis of copay and foundation assistance if required when we do a thorough benefits investigation.
- Assigning case managers to patients up front to evaluate potential barriers to care, including financial ones. Patients are evaluated for available financial assistance, and we assist patients in applying for manufacturing programs.
- Automatically reviewing opportunities for financial assistance in real time—and case managers are made aware of these opportunities in real time so they can assist patients with applying for assistance as soon as possible.
- Providing real-time order status updates to prescribers. For manufacturers, we provide clinical data collection and reimbursement obstacle support.
- Having a streamlined referral process that is easy to implement. We customize the process according to the prescriber’s preferences and make sure we are communicating about receipt of information and barriers to care. If we cannot service a patient, we send these patients to a pharmacy that can service them while updating all stakeholders on care transitions. - Providing caregiver support by providing a custom caregiver guide that includes support resources and recommendations on easing treatment burden. In addition, we work cross functionally with multiple providers. We call payers, providers, and other entities to advocate on the caregiver’s behalf, and our experienced billing team follows a thorough process to help identify Medicare coverage.
